PATIENT'S RESPONSIBILITIES
·
To keep
scheduled appointments or call to reschedule within 24 hours of appointment
·
To follow
instructions of the health care providers to the best of their ability in order
to obtain best health outcomes
·
To inform
providers or management about any problems or concerns related to care or
treatment received at the health center
·
To keep
health center staff informed of changes in address, contact numbers, insurance
coverage or financial status
·
To provide
health center with proof of income if cost of services are to be discounted
·
To pay for
services at time they are rendered. If not possible, speak to case manager or
financial staff to make payment arrangements.
